The Trinity Metro Medical Clinic will provide employees and their eligible dependents convenient access to comprehensive healthcare and wellness services. Integrated medical services, technology, and robust analytics allow our team of
dedicated staff to focus on high-quality personalized preventive care and effective management of chronic conditions.

Premise Health is the organization that operates the Trinity Metro Medical Clinic. They are the world’s leading direct healthcare provider. Premise delivers an effortless member experience that raises the bar, lowers costs, and redefines the
meaning of quality care.

The Medical Clinic will be located at 1600 E. Lancaster Ave., Fort Worth, TX 76102.

All employees (regardless of medical coverage) and dependents ages 2 and up who are enrolled in Trinity Metro’s medical plan.

The Medical Clinic will offer services including, but not limited to:
• Annual Physicals
• Preventive Exams
• Biometric Screenings
• Lab Draws
• Allergy & Flu Shots
• Immunizations
• Acute & Urgent Care
• Women’s Health
• Wellness Coaching
• Pediatric Care

You can request that your PCP share your records with the Medical Clinic by completing your provider’s record sharing process. Additionally, you may request your records from Premise Health be shared with your PCP or specialist.

No, the Medical Clinic will be staffed and operated by Premise Health. Premise Health will manage the Medical Clinic just as any licensed medical office in the community.

Premise Health has recruited a high-quality team of dedicated healthcare professionals to staff and manage the Medical Clinic. The team will include a Nurse Practitioner and Medical Assistant.

Your personal health information will remain confidential and your data will be kept secure. The Medical Clinic will operate in accordance with HIPAA and will work diligently to protect all health records. Premise Health can’t share any personal health information without your permission. Your personal health information will not be shared with Trinity Metro. Notice of privacy practices will be available for review within the Medical Clinic.

All services are free to employees and dependents enrolled in the medical plan. If you are not covered under the medical plan, you will be responsible for all costs associated with your visit. For a more detailed breakdown of costs and services, ask your Premise Health care team.

When the Medical Clinic opens in September, you will be able to make personal appointments in a variety of ways:
• Call or visit during regular operating hours to make an appointment.
• Use the online member portal, My Premise Health, to make your appointments via your computer, phone or tablet. More details to come on how to register.

My Premise Health is a secure online and mobile tool that connects you with your healthcare experience by providing convenient virtual visits, access to your providers, health records, visit history, test results and more.
